I enroll the children listed above in Mt. Calvary Kids Vacation Bible School.  I give permission for participation in field trips (You will be notified prior to any trip).  I give permission for emergency medical treatment for the children named above if it is deemed necessary while attending Mt. Calvary Kids.  I also give permission for the child/children participant to be photographed and/or video recorded for promotional or social media purposes.  By filling in the signature box below, I accept that this will serve as my signature for the electronic document or record.
